A mine worker died shortly after coming into contact with a heavy conduit.

Authorities recently examined how a crew member became fatally struck by a pipe at an undisclosed mine on 1 March 2024.

“A miner died after a metal slurry pipe struck him. The miner was removing the last bolt connecting two metal slurry pipes when the pipe broke free and swung in his direction,” the Mine Safety and Health Administration said in a safety alert.

Investigators discovered the victim failed positioned himself in a way that avoids hazards.

They made the following recommendations:

  • examine work areas before and during the shift for hazards
  • train employees in safe work procedures and hazard recognition
  • monitor personnel routinely to ensure they follow safe work procedures
  • ensure that blocking material is competent, substantial and adequate to stabilise the load.
